    Some locations times & costs could change subject to final ice allotment granted by city. All costs include increase of ice costs

    Before players may step on the ice, EVERY PLAYER MUST provide a complete Amateur Athletic Waiver & Release of Liability Form. This form is your ‘ticket’ onto the ice. Players may print off and bring a completed copy to the rink themselves by clicking the link below. Forms will also be provided at the first skate to be completed. By placing a deposit or paying in full, you are agreeing to the terms outlined in our Amateur Athletic Waiver & Release of Liability PDF Form (http://worldhockeycentre.ca/forms/waiver.pdf).

    *PLEASE NOTE* If you wish to only place a $215.00 deposit to secure your registration, rather than paying in full, please call the office at 905.775.0062 and we can accomodate the request I understand and accept that there are risks of serious injury in connection with participation and voluntarily assume and accept those risks. I also warrant that I am medically cleared to play this sport. By submitting this registration form, I unconditionally release Russell Hockey Ent. affiliates, coaches, sponsors, volunteers and employees from any and all suits, claims and demands of any kind for personal injuries, property damage that I may sustain while participating in the hockey and related activities and accept to all conditionds in the Russell Hockey Ent. Waiver. Only the person registering must submit the form if participant is over 18 years of age.
